The biggest helps I've found was to set the character listening distance to 100 (right on character, no camera). I only had to change that once. But every time I log in I have to change the audio preset from Standard to any other option and applying, then back to Standard and applying.

Check your audio settings. If you have surround sound enabled but don't actually have a surround speaker set up, the game tries to put the voice acting channel to a speaker that doesn't exist.
